When downloading and completing these forms, you must:

1. Open and complete in Adobe Acrobat. Do not use any other program to complete this form.
2. If you don't have Adobe Acrobat Reader please download it for FREE here https://get.adobe.com/uk/reader/
3. Once Acrobat has installed, then download the attachments/downloadable forms (by selecting the save all attachments option) to your chosen area e.g. your desktop.
4. When the files have been saved to desktop right-click the file and select the option Open with > Adobe Acrobat Reader DC.
5. The form will then open in Acrobat then save the file.
